At CTC Recovery, your story is yours — and your personal health information deserves to be treated with the care and respect you expect.
This page explains how we protect your information, when we may share it, and what rights you have. We're required by law to tell you this, but we also believe you deserve to understand it clearly — no fine print, just the facts.
What is “PHI”?
PHI stands for Protected Health Information — that’s any information about your health, treatment, or payment that can identify you. It includes things like:
- Your medical history
- Diagnoses or treatment plans
- Insurance or billing info
Why We Might Use or Share Your Information
We’ll only use your PHI for reasons that support your care or are required by law. Most commonly, that means:
1. Treatment
To help provide and coordinate your care across our team — like your therapist, doctor, or case manager.
2. Payment
To bill your insurance or verify coverage so you can receive care.
3. Operations
To run CTC Recovery efficiently — things like staff training, audits, or improving our services.
Situations Where We May Share Without Asking First
There are a few situations where we might be required to share your info, even without asking. These include:
- Emergencies (if you're unable to give consent)
- Legal obligations (court orders, reporting abuse, etc.)
- Public health and safety (like reporting a serious health risk)
- Government requests (military or national security)
We will always do our best to only share what’s absolutely necessary.
Family & Friends Involved in Your Care
If someone you trust (like a family member or friend) is helping support your care, we may share relevant information with them if you give us the okay — or in emergencies, if you're unable to speak for yourself.
When We Need Your Written Permission
There are some situations where we’ll always ask for your written consent first, like:
- Sharing your info with outside providers not involved in your care
- Using your info for marketing or fundraising
- Posting any testimonials or personal stories
And if you change your mind later? You can withdraw your permission at any time, in writing.
Your Rights
You have full rights over your information. Here's what that means:
📥 Access
You can ask to see or get a copy of your records.
📝 Amend
You can request corrections if you believe something is missing or incorrect.
📃 Disclosure List
You can ask for a list of when we’ve shared your PHI (not including treatment, billing, or operations).
🚫 Restrict Sharing
You can ask us not to share with certain people or groups — and we’ll honor your request unless we’re legally required to share.
📬Communication Preferences
You can request we contact you in specific ways — like by email, text, or at a different address.
